Earn My Health Pays rewards when you complete healthy activities like a yearly wellness exam, annual screenings, tests and other ways to protect your health.

Use Your My Health Pays rewards to help pay for:

Or, you can use them to:

  • Shop at Walmart for everyday items**

**Rewards may not be used to buy alcohol, tobacco, or firearms products.

My Health Pays Rewards Card

Your My Health Pays rewards are added to your My Health Pays® Visa® Prepaid Card* after we process the claim for each activity you complete.  If you are earning your first reward, your card will be mailed to you.

 *This My Health Pays® Visa® Prepaid Card is issued by The Bancorp Bank, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Card cannot be used everywhere Visa debit cards are accepted.

2026 Healthy Activities Eligible for Rewards
  • $5 Member Portal Registration-Visit OklahomaCompleteHealth.com and click “Login”.
  • $10 Health Risk Screening. Once per enrollment.
  • $10 Flu Shot. Once per year. 
  • $10 Cervical Cancer Screening. Ages 21-64. Once every three years. 
  • $10 Colorectal Cancer Screening. Ages 45-75. Once per year. 
  • $10 Diabetic Eye Exam. Ages 18-75. Must have a current diagnosis of diabetes to qualify. Once per calendar year. 
  • $10 Diabetic Kidney Screening. Ages 18-85. Must have a current diagnosis of diabetes to qualify. Once per calendar year. 
  • $20 Behavioral Health Post Hospitalization Discharge Visit. Ages 6-17. Must be within 30 days of discharge date. 
  • $20 Behavioral Health Post Emergency Room Visit. Ages 6+. Must be within 30 days of emergency room visit. 
  • $25 Notification of Pregnancy Form. Completed within first trimester. 
  • $25 Child Well Visit. Ages 3-20. Once per year. 
  • $25 Adult Well Visit. Ages 21+. Once per year. 
  • $25 Diabetes HbA1c Test. Ages 18-75. Must have a current diagnosis of diabetes to qualify. Once per calendar year. 
  • $25 Diabetes Blood Pressure. Ages 18-75. Must have a current diagnosis of diabetes toqualify. One blood pressure test per calendar year.
  • $25 First Fill of Tobacco Cessation Medication.
  • $25 Immunization for Adolescents (IMA). Ages 10-13. Requires one dose Mengingococcal, one dose Tdap, and 2 doses HPV. One reward per lifetime. 
  • $25 Childhood Immunization Status. Ages 0-2. Requires completion of dose series. One reward per lifetime. 
  • $25 Controlling Blood Pressure. Ages 18-85. One blood pressure test per calendar year. 
For Pregnant and New Moms
  • $10 Notification of Pregnancy Form. Completed within second trimester. OR $25 Notification of Pregnancy Form. Completed within first trimester. 
  • $25 Prenatal Visit. Once per pregnancy. 
  • $25 Postpartum Visit. Once per pregnancy within 7-84 days of delivery. 
  • $25 Infant Annual Well Visit. Ages 0-15 months.  Earn one reward for each visit. Child can receive up to six well child visits prior to reaching 15 months old. 
  • $25 Infant Annual Well Visit. Ages 15-30 months.  Earn one reward for each visit. Child can receive up to six well child visits prior to reaching 30 months old. 

* To be eligible for this reward, you must notify us you are pregnant prior to having your baby by submitting a completed Notification of Pregnancy (NOP) form.

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact Member Services.

Once you receive your My Health Pays rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation. 

To check your balance, login to your Secure Portal account.

Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
