OKLAHOMA COMPLETE HEALTH PARTNERS WITH OU SOONER HEALTH ACCESS NETWORK TO IMPROVE ASTHMA CARE FOR SOONERSELECT MEMBERS

Oklahoma Complete Health is launching a new partnership with the OU Sooner Health Access Network (HAN) to deliver targeted care management services for SoonerSelect members diagnosed with asthma. This initiative reflects a strong commitment to collaborating with local Oklahoma provider organizations and enhancing our members’ care management experience.

The OU Sooner HAN has spent more than a decade building a complex care management system to support primary care practices and patients across Oklahoma. The program earned National Committee for Quality Assurance (NCQA) accreditation for its Complex Case Management Program three years ago, making it the first of its kind in the state.

“This partnership is about leveraging our system-wide infrastructure and Sooner HAN’s established relationships,” said Oklahoma Complete Health Vice President Population Health Dr. Keisia Sobers-Butler. “By working with trusted local providers, we can ensure our members receive local and personalized support they need to manage chronic conditions like asthma.”

Members will be categorized into three levels of risk and will receive comprehensive care management services designed to improve health outcomes and reduce avoidable hospital visits. Sooner HAN care managers work seamlessly with members, their families or caregivers, and primary care providers to complete assessments, set goals, and develop personalized treatment plans that support better health outcomes.

Key services offered by Sooner HAN include:

  • Care Management: Proactive outreach, health assessments, personalized care planning and ongoing follow-up
  • Disease Management: Asthma education, medication support and tools for self-care
  • Care Coordination: Collaborate with PCPs, specialists, behavioral health professionals and community organizations. Administer value-added services, including the Breathe Better at Home asthma management program helping members manage chronic conditions in a safe and supportive environment.
  • Transitions of Care: Support during changes in care settings to ensure continuity and minimize disruptions

Sooner HAN will operate independently, working directly with members to conduct health risk screenings, develop care plans, and report clinical outcomes to the health plan. Members will continue to have access to all value-added benefits.

“We’re excited to begin this focused rollout for Oklahoma Complete Health SoonerSelect members,” said Rachel Mix, Director of the University of Oklahoma Sooner Health Access Network. “With the transition to SoonerSelect, this partnership strengthens our ability to improve health outcomes while ensuring Oklahomans continue to care for Oklahomans.”

Sooner HAN will also support diabetes education and health coaching for Oklahoma Complete Health’s Children’s Specialty Program members further expanding the reach of this collaborative model.
